A new article from Reuters yesterday on the benefits of mammograms and how their screening is even greater than once thought. We thought this was such an important article on the longest running study of breast cancer screenings, that we wanted to share it and its information with you. (Reuters) - The longest-running breast cancer screening study ever conducted has shown that regular mammograms prevent deaths from breast cancer, and the number of lives saved increases over time, an international research team said on Tuesday.

The study of 130,000 women in two communities in Sweden showed 30 percent fewer women in the screening group died of breast cancer and that this effect persisted year after year.

Now, 29 years after the study began, the researchers found that the number of women saved from breast cancer goes up with each year of screening.

"We've found that the longer we look, the more lives are saved," Professor Stephen Duffy of Queen Mary, University of London, whose study was published in the journal Radiology, said in a statement.

Dr. Stamatia Destounis, a radiologist at Elizabeth Wende Breast Care in Rochester, New York, who was not involved in the study, said radiologists have been quoting results of the Swedish study for years and the new findings show breast cancer screening is "even more of a benefit than we understood."

She said sweeping changes in the U.S. screening guidelines two years ago that scaled back recommendations on breast cancer screening caused a lot of confusion among doctors and patients about the benefits of mammograms.

"We've had to do a lot of education of the patients and their doctors. This will help for that," Destounis said.

In the study, women were divided into two groups, one that received an invitation to have breast cancer screening and another that received usual care.

The screening phase of the trial lasted about seven years. Women between 40 and 49 were screened every two years, and women 50 to 74 were screened roughly every three years.

"Our results indicate that in 1,000 women screened for 10 years, three breast cancer deaths would be prevented," Duffy said, adding that most of the deaths prevented would have occurred more than a decade after the screening had started.

"This indicates that the long-term benefits of screening in terms of deaths prevented are more than double those often quoted for short-term follow-up."

The new data adds to evidence on the long-term benefits of regular mammography screening.

SCREENING CONTROVERSY

New breast screening recommendations issued in 2009 by the U.S. Preventive Services Task Force, an influential advisory group, recommended against routine mammograms for women in their 40s and said women in their 50s should get mammograms every other year instead of every year.

The guidelines contradicted years of messages about the need for routine breast cancer screening starting at age 40, eliciting protests from breast cancer experts and advocacy groups who argued the recommendation for fewer screenings would confuse women and result in more deaths from breast cancer.

The changes were meant to spare women some of the worry and expense of extra tests needed to distinguish between cancer and harmless lumps. But the latest results from the Swedish study show the rate of false positive results was low.

"We saw the actual number of overdiagnosed cases was really very small -- less than 5 percent of the total," Robert Smith, director of cancer screening at the American Cancer Society and one of the study's authors, said in a telephone interview.

Many groups, including the American Cancer Society, have stuck by their long-standing recommendations of a yearly breast exam for women starting at age 40, stressing that the breast X-rays have been proven to save lives by spotting tumors early, when they are most easily treated.

"I think for anybody who was beginning to have their faith shaken in the value of mammography, these data show mammography is quite valuable as a public health approach to reducing deaths from breast cancer," Smith said.

Duffy said he thinks screening women 40 to 54 every 18 months and screening women 55 and older every two years would be a reasonable schedule.

He said the new findings do not speak to the frequency of screening issue, but they do make clear that screening works.

"Everyone must make up their own mind, but certainly from combined results from all the screening trials, mammography in women aged 40-49 does reduce deaths from breast cancer," he said.

Breast cancer is the second-leading cause of cancer death among U.S. women, after lung cancer. It kills 500,000 people globally every year and is diagnosed in close to 1.3 million people around the world.

What does MRI do?

MAMMOGRAPHY
Do not use deodorants, dusting powders or perfumes on the day of your exam. Many patients find it more comfortable to refrain from caffeine products for 3 days prior to a mammogram.

CT SCANS
You may take medications prior to exam with a small amount of water

See below for specific exam preps:

HEAD - no eating or drinking for 4 hrs prior to your scan
SINUS - No restrictions
DENTAL IMPLANT STUDIES - No restrictions

You will not have to undress for the above CT exams.

NECK - no eating or drinking for 4 hrs prior to your scan
CHEST -no eating or drinking for 4 hrs prior to your scan
ABDOMEN - no eating or drinking for 4 hrs prior to your scan
PELVIS - no eating or drinking for 4 hrs prior to your scan
SPINE - no restrictions
EXTREMITIES (arms/legs) - No restrictions

If you wear clothing with no buttons, snaps or zippers, you will not have to undress for your exam

BONE DENSITOMETRY (DXA)

Must be done more than 5 days after other imaging exams using contrast - ie: CT scans, nuclear medicine tests, barium studies.

If you wear clothing with no buttons, snaps or zippers, you will not have to undress for your exam.

GENERAL RADIOGRAPHY
No restrictions prior to plain x-ray exams

UGI/SMALL BOWEL EXAMS - Nothing by mouth 12 hrs. prior to exam (essential medications may be taken with a small amount of water a minimum of 2 hours prior to exam)

If you wear clothing with no buttons, snaps or zippers, you will not have to undress for your exam.

MAGNETIC RESONANCE IMAGING (MRI/MRA)

You may take medications prior to exam with a small amount of water
See below for specific exam preps:

• MRCP - no eating or drinking for 4 hrs. prior to your scan
• ABDOMEN MRI - no eating or drinking for 4 hrs prior to your scan
• ALL OTHER MRI/MRA EXAMS - No restrictions

ULTRASONOGRAPHY (US)
You may take medications prior to your exam with a small amount of water.

ABDOMEN, GALLBLADDER or LIVER
No eating or drinking for 6 hours prior to your exam

PELVIS or BLADDER
Arrive with a full urinary bladder (drink 4 – 12oz. glasses of fluid 1 hour prior
to your exam time – do not urinate)

CAROTID, EXTREMITIES (ARMS/LEGS), THYROID, PROSTATE, BREAST,

CT Scan Saves Life

At Capital Imaging we take our jobs very seriously. We never know what may happen on any given day. Case in point, on January 17th a female in her 50's arrived for a CT scan. Upon review of the CT results, it was discovered that she had multiple pulmonary emboli, which is an extremely dangerous and potentially life-threatening condition. Our Radiologist made the decision to transport her to the hospital via ambulance for medical intervention.

MRI Q and A with Capital Imaging

Welcome to the first installment of MRI Q and A With Capital Imaging. In this series, you will receive information on the sort of questions that we and our technicians have to ask patients before they have an MRI. We hope that this information is useful to you and might alleviate any stress you may feel before experiencing an MRI of your own.

MRI Q and A with Capital Imaging:

We have 2 types of contraindications one is absolute contraindication which the patient can never have MRI other are contraindications in which further information is needed about the patients condition when these questions have been answered the patient may proceed with an MRI.

1. Does the patient have a cardiac pacemaker:

If the patient has a cardiac pacemaker they may not have an MRI.

For that matter, if the patient has known serous metal, meaning metal that is attracted to magnet in their body either on purpose or an accident, the patient may not have an MRI.

2. Does the patient have tattoos of any kind?

We also ask patients if they have tattoos. The reason for this is that many tattoo inks, especially in inks that were used some time ago, may have metal filings in the ink that may act adversely in the magnetic field. There have been reports that some tattoos, after being exposed to the magnetic field, have become raised and reddened. It has also been reported that usually the color red as more susceptible to this type of reaction. So be sure to be sure to let your technician know about any tattoos you may have before receiving an MRI.

Ultrasound, also referred to as sonography, is a medical imaging technique that uses high-frequency sound waves to create high resolution images of the inside of the human body. It is painless and uses no radiation. Dear Capital Imaging,

I was wondering if I should be concerned about radiation levels when getting an MIR? I have an apt to get one next week and had this concern.

Dear Michele,

Great question, many people think that MRI uses radiation, but the good news that is does not!

MRI uses high frequency radio waves (near or in the FM band) and a strong magnetic field. So there is no need to be worry about radiation exposure. These energies at the levels used are deemed safe to us.

There are some contraindications to having an MRI however, like having a cardiac pacemaker or some kind of implanted pump (designed to administer medication), or having metal in your body… braces, most piercing and prosthetic implants(knee replacements) are safe. But if you have any of these be sure to mention it to your technologist prior to the exam so they can make sure.

Lower Dose from FFDM Confirmed For Mammography At Capital Imaging Associates And What It Means To You

My name is Michael Masone, R.T. (R) (M) (MR) (S), RDMS. I am the Director of Operations at Capital Imaging Associates and am a certified technologist in Radiography, Mammography and MRI as well as holding a dual certification in Ultrasonography.

The purpose of this blog is to create a public forum to post and share information and hopefully answer your questions about medical imaging studies.

In the way of background information, Capital Imaging Associates was the first freestanding imaging center and has been providing medical imaging services to the capital district since 1985. As the first, we brought magnetic resonance imaging (MRI) to the upstate New York region and many of our staff members are celebrating their 25th anniversary with us. Our team was instrumental in educating referring physician offices and the community on the benefits and safety concerns of MRI scanning. In addition, we provided vital information on the uses and medical necessity of MRI to the insurance companies, facilitating payment for this service on behalf of our patients.

Some of the questions we have been asked over the years include

· Why did my doctor order this exam versus another?
· Why does my insurance company now require prior approval for my exam?
· What is the National Imaging Association (NIA) and why does it have control over my imaging procedure?
· Are there any harmful effects from having an MRI exam?
· How much radiation is there in a DXA scan?
· What makes me at high risk for breast cancer?
· When should I start my screening mammogram tests?

With two and a half decades of experience to draw upon, we feel we can be a valuable resource and hope you will use us as your reference site for medical imaging procedures.

The following article, which was published in the March/April Journal of The American Society of Radiologic Technologists, addresses radiation dosage with digital mammography.

The American College of Radiology Imaging Network (ACRIN) recently announced results of a study comparing full-field DIGITAL mammography (FFDM) with film-screen mammography. Researchers compiled data on 5102 patients at 33 sites who were participants in the Digital Mammography Imaging Screening Trial (DMIST).
Differences in compression force and compressed breast thicknesses were minor between FFDM and Film-Screen mammography. Although 21% of FFDM cases required more than 4 views compared with 12% of the film-screen cases, mean glandular dose per patient was 17% lower for those undergoing FFDM than film-screen mammography. Results of the ACRIN DMIST study comparing acquisition parameters and breast dose appeared in the February 2010 issue of AMERICAN JOURNAL of ROENTGENOLOGY.

This is great news for patients who utilize digital mammography technology, but even more so for patients who have their mammograms done at Capital Imaging Associates. Here’s why …

The study, which was originally published in the February 2010 issue of the American Journal of Roentgenology, reported that digital mammography not only improved the image quality of the mammogram, but it also DECREASED the amount of radiation the patient received, even though 21% of patients receiving full field digital mammography required more than four views per exam. This is because first generation digital mammography units had a relatively small surface to place the breast on. As a result, even some average-sized patients required more than two images per side in order to capture the entire breast on one exam. The newer digital mammography units, such as the one here at Capital Imaging, have a surface almost twice the size of the earlier units which decreases the images needed to capture the breast in most cases. While 21% of patients needed additional images in that study, our experience has been less than 2% of patients require more than four images per exam, thus decreasing the amount of radiation even more.

In addition to less radiation exposure to our patients, our technology incorporates biofeedback elements which enable us to provide our patients with a more comfortable experience. You can read more about this on our website under the mammography tab.
